There was a thread investigating this (yes) a few weeks ago. The furthest they got is that around 2004-2005ish (could be wrong on the date, but before 2009) some camwhore posted a picture of herself on /b/ asking if she was pretty. Somebody shopped the image into OP's pic (or the other one, not sure which was first) saying "now you are". After a few years someone took the shopped pic and named it Jeff The Killer.

My theory is that with the original image her face was erased and made essentially a blank canvas for anyone to manipulate.
I think the eyes could have been made in photoshop but the mouth definitely came from something else, it's really creepy.
